ABOUT US
Disease Management is recognized as a multidisciplinary approach to treating people living with chronic diseases through the use of coordinated interventions, integrated care, and evidence-based guidelines. Examples of chronic disease diagnoses or conditions that can improve through Disease Management are diabetes, asthma, heart failure, COPD, renal disease, and depression. Through the implementation of Disease Management principles and education, outcomes such as reduction in medical costs or services, improved quality of life, and patient empowerment and self-management can be achieved.

- Assist in scheduling and keeping medical care appointments
- Healthcare Home
- Limited van transportation
Two ReDiscover programs, DM3700 and Health Care Home, were both established by the state and implemented in Missouri’s Community Mental Health Centers to aid clients who live with mental illness and chronic disease in improving their quality of life.

Who We Serve:
ReDiscover’s Disease Management program serves Missouri residents and accepts Medicaid, Medicare, private insurance, and self-pay clients. Individuals in need of financial assistance and out-of-state residents with private insurance are able to seek services.
