INTENSIVE SUPPORTIVE HOUSING FOR ADULTS WITH SEVERE AND PERSISTENT MENTAL ILLNESS
ReDiscover’s Cluster Apartment Program (CAP) provides supportive housing for adults as an alternative to long-term psychiatric hospitalization. CAP staff provide intensive wrap-around support and medically necessary on-site services for individuals who have a history of failure in multiple community settings and/or the presence of an ongoing risk of harm to themselves or others. Treatment provided may include medical, dental and mental health care, vocational activities, help promoting and fostering engagement in social and recreational activities, and assisting with nutrition management, and harm reduction. The goal is to house the ‘unhousable’ while teaching the necessary skills for independence to sustain their housing and improve their quality of life.
Please note that houselessness is not enough of a criteria for CAP, but we do have a separate Respite Apartment that is intended to rehabilitate members of the houseless community.
Our program serves any clients that meets CPRC guidelines, specifically those with limited functional skills and those who have never lived independently or have failed due to repeated MH related issues and therefore benefit from wrap around intensive support.
Who We Serve:
ReDiscover’s Cluster Apartment Program serves Missouri residents who live with severe mental illness or a disability that prevents them from being able to successfully live on their own, but do not require 24-hour observation.
