AN ALTERNATIVE TO MENTAL HEALTH HOSPITALIZATION
ABOUT US
ReDiscover’s Thrive Center offers an alternative to mental health hospitalization. We provide medication stabilization and residential hospital diversion for those in crisis.
Thrive is dedicated to supporting individuals with mental health disorders, life crises, houselessness, and substance use disorder. We offer top-notch clinical treatment and 30 days of follow-up services post-discharge to ensure a smooth transition back into the community and ongoing support for mental health and housing needs.
Program Services:
- Community Support services
- Individual and group therapy
- Medication management
- Community placement in affordable housing in the least restrictive environment with meals provided
- Connection to ongoing community mental health services through partnership with surrounding
community mental health centers - Management of any presenting medical needs
- Follow-up care for 30 days post discharge with intensive Community Support services
We work closely with clients to form a treatment plan with their specific goals and needs in mind. We provide connection to community services, affordable housing, and address any medical care issues that may be part of the crisis situation.
We offer warmth, empathy, and compassion with a wide range of recovery-oriented services.
We help clients stabilize by providing assistance with legal issues, housing barriers, and getting access to missing documentation, as well as any additional goals clients have set for themselves.
Eligibility:
- Adults ages 18 and older
- Mental health and/or substance use crisis
- In need of medication stabilization
- No major medical needs.
- Able to self ambulate
