It targets individuals who are high utilizers of emergency services (ERs, hospital, law enforcement, EMS, and other emergent services).<\/p>\n<p>ReDiscover\u2019s Hospital Diversion Program works with referred clients to:<\/p>\n<ul>\n<li>Increase access to appropriate resources Remove barriers to treatment<\/li>\n<li>Reduce unnecessary use of Emergency Rooms, hospitals, law enforcement and EMS<\/li>\n<li>Improve mental and physical health Decrease homelessness<\/li>\n<li>Increase stability in the community<\/li>\n<\/ul>\n<p>ReDiscover leads this initiative.
